Logan Health Community Healthcare Worker - Case Management - Eureka (Part-time 20 hrs/wk) in Eureka, Montana

Job Description Summary:

This position strives to improve health outcomes for patients with complex social and medical issues. Assists patients in connecting to community resources in an effort to help regain health and independence.

Qualifications:

  • Minimum of two (2) years’ experience in a primary care setting, medical office, healthcare or social services role preferred.

  • Knowledge and understanding of social service programs (e.g. Medicare, Medicaid, VA, etc.) preferred.

  • Possess knowledge of community resources/programs to support and meet socio-economic needs, such as disability income, food, shelter, transportation and health coverage preferred.

  • Current Drivers’ License required. Must possess a good driving record clear of moving violations in the past five (5) years and no major driving offenses (DUI, etc.).

  • Possess and maintain computer skills to include working knowledge of Word, Outlook, Excel, and ability to learn other software as needed. Experience in record keeping and data entry preferred.

  • Possess a clear understanding of the confidentiality laws that govern the provider-patient relationship, including but not limited to HIPAA guidelines preferred.

  • Excellent organizational skills, detail-oriented, a self-starter, possess critical thinking skills and be able to set priorities and function as part of a team as well as independently.

  • Commitment to working in a team environment and maintaining confidentiality as needed.

  • Excellent verbal and written communication skills including the ability to communicate effectively with various audiences.

  • Excellent interpersonal skills with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy.

Job Specific Duties:

  • Bridges the gap between communities and the health/social service systems to assist clients in navigating and connecting to resources to improve health and welfare.

  • Builds relationships with the client by listening, identifying barriers and developing effective self-management skills.

  • Investigates and researches potential solutions for identified concerns related to finances, food, shelter, transportation, etc.

  • Utilizes communication skills to share solutions and encourage clients to regain health and independence.

  • Coaches clients on when to utilize medical services based on medical level of need: primary care physician, urgent care, and/or emergency room.

  • Supports the Care Continuum Team by reinforcing approved medical and dietary educational training, and facilitates in home assessments via telephone or telehealth measures.

  • Conducts basic home safety assessments as directed by the care team. Documents all visits and services provided into appropriate database to track patient care.

  • Stays abreast of community resource programs and effectively assists clients to connect and regain their health and independence.

  • Assists clients to complete forms (ie: for insurance programs, housing, food assistance, financial aid, etc.). Follows-up to ensure the applications are complete and contain all support documentation and continues with follow-up through finalization.

  • Assists clients with coordination of scheduled appointments and attends appointments and takes notes when appropriate.

  • Collaborates with appropriate stakeholders for optimal care coordination. Communicates with providers, caregivers and clients in order to coordinate services.

Please know schedules and shifts are subject to change based on patient care and department/organizational needs.

20 hours a week; Day Shift - Varied Hours (Depending on candidate availability).
